Patient Card Pilot


This project initiated by a group of our GP mentors is a small pilot running across Rugin, Bichhya and Jair in the far west of Nepal. GPs and ANMs (Auxiliary nurse midwives) have noticed that it isn’t always easy to notice trends and understand the history of all of their patients. In order to increase the quality of service nurses are able to deliver to the community we have worked together on introducing a pilot of patient held record cards. The Government of Nepal have an ambition to roll out an electronic system across the whole country, but with limited access to technology and power, this is unlikely to get to Bajura and Humla for some time.

The main aims of this initial project are to:

  • Identify whether patients value the cards and they remember to bring them to the health post.
  • Identify in health staff both PHASE and government continue to add information to the cards.
  • Establish a system for monitoring who has been issued with a card and whether they are retained and patients understand they need to bring them to each appointment.

The cards were printed in April 2026 and are now in health posts. The first review will be in November 2026 to see what is going well, what isn’t going well and if there are any adjustment that need to be made to the project.

Our ambition once we have refined the project to issue health cards in all PHASE communities.
